Here we go again solely

Here we go again solely blaming the company for the fatality; these day you cannot just pass an HGV (LGV) test and drive a truck you also need to pass a CPC (Certificate of Professional Competence) so why is this not considered? Furthermore it does not take a brain surgeon to work out that objects with wheels will roll down hill; the trailer would have had a mechanical brake so why was this not applied? All we appear to get is a point the finger at the company and blame them approach, the HSE always say that the accident could have been avoided but reality says differently or else these tragedies would not take place at all, when does some or all of the blame lay on the individual and not the company, surely he was a skilled driver who should have known to check the parking brake was applied.
