A really good article for all SHE investigators to read and think about. Currently 'the blame game' and NIMBY approach corporately, is an issue. Impartial approach is necessary, gathering all facts (statements/photos/CCTV/RAMS/Attitudes, to you by all, when meeting them for the first time), look at timelines and build a picture of how the work should have proceeded, what appears to have happened, the site leaders teams culture (behaviour/attitude/stance) and assess the facts, be impartial. Results should be 'development opportunities' for all for non fatal occurrences to effect change with actions/timescales by leaders for a culture change
